Your Appointment

We typically offer appointments Monday through Friday. We closely monitor appointment availability and requests, and adapt our schedule to meet our patients' needs.

You can expect to receive automated phone calls, emails, or text messages to remind you of your appointment, to let you know that you're due for an appointment, and to alert you of scheduling changes.

Below are the forms needed for your appointment. Please print them and complete them, and bring them with you to your appointment. If you're able to fax them to us in advance, please send them to 513.333.3024 and bring the originals to your appointment.
